Appointments
To Make an Appointment
Please call Dr. Benton at 918-392-4866, option 4 to schedule an appointment. Messages should be returned within 24 hours, unless otherwise noted on the voicemail message.
Paperwork will need to be completed for each new patient (and previous patients who have not been seen in the last 12 months or at this office), but it will be provided at the first appointment.
Please bring the following to your first appointment:
- A photograph of the patient (that will be kept in the chart)
- Your insurance card
- A photo ID for the patient or patient’s parent
- Previous psychological/educational testing (if applicable)
- Individualized education plan (if applicable)
Cancellation Policy
If you do not attend your scheduled therapy appointment, and you have not notified Dr. Benton at least 24 hours in advance, you will be required to pay a “no-show” or late cancellation fee.
